M.A.D. Duck Information

M.A.D. Duck is an award recognizing those who are Making A Difference at Lone Star College–CyFair.  We have all seen employees who go above and beyond their normal job duties to enhance outstanding customer service to our coworkers, students and the community!

Any employee (faculty and staff) of Lone Star College–CyFair who have demonstrated outstanding customer service by going above and beyond their normal job duties.

Click on the link below, complete the nomination form and e-mail the form to cy-fair.madduck@lonestar.edu. It’s that simple!

Nominees will be honored on the second and fourth Tuesday of each month, and if you like, as a nominator you can personally deliver the nomination!  Just check the appropriate box on the Nomination Form.

Each month an outstanding M.A.D. Duck nominee will be selected for the monthly prize - a $25 American Express Gift Cheque; and have their picture taken with Daphne the Lone Star College–CyFair M.A.D. Duck!

-Detailed Instructions-

1. Click on the Nomination Form, and SAVE the form on your computer (Somewhere you can find it easily).
2. Open the Nomination Form & fill in the shaded fields.
3. Once comleted SAVE the nomination form & close it.
4. In Outlook, click on "New Mail Message"and proceed to attach the Nomination Form (Which you saved on your computer).
5. Address the Email to EXACTLY the following name:  cy-fair.madduck@lonestar.edu.
6. Once you have ensured the correct e-mail address, and properly attached the Nomination Form, click the "Send" button.
7. Immediatly after your e-mail was successfully sent, you will receive an automatically generated Thank You message verifying the receipt of your Nomination Form
